Walk into any boardroom this year and someone is talking about AI. The deck looks great, the pilot demo runs cleanly, and the executive sponsor nods along. Six months later? Nobody really brings it up anymore. The pilot is “on pause.” Or it shipped, technically, and nobody’s using it.
This pattern is shockingly common. It’s not popular to say this out loud, but most enterprise AI projects don’t fail because the model is bad. They fail for boring reasons. Process reasons. People reasons.
A lot of organizations end up bringing in an ai solution provider only after the in-house experiment has already wobbled, which is sort of backwards. Anyway.
1. The Problem Was Never Really Defined
This one’s almost too obvious to mention. Except it keeps happening.
Teams launch with vague missions. “Use AI to improve customer experience.” Okay. Improve which part? For whom? Measured how? In many cases the technical team interprets the brief one way and the business sponsor meant something completely different. Nobody catches it until somebody in a steering committee asks what success actually looks like.
An MIT report from last year put the figure at around 95% of generative AI pilots producing no real P&L impact. Ninety-five percent. The researchers traced most of it back to enterprise integration and the “learning gap” rather than the model itself. Which, fair enough.
2. Pilots Live in a Bubble
Pilots are clean. Production isn’t.
In a pilot, you get curated data, hand-picked users, and a project manager basically chaperoning the whole thing. Then you try to scale it and suddenly you’re dealing with the legacy CRM, three different ways the sales team enters customer data, and a security review that takes ten weeks. That gap between “it worked in the demo” and “it works on Tuesday at 3pm for everyone” is brutal.
3. Governance Gets Bolted On Late
Most companies treat AI governance the way they used to treat cybersecurity. Skip it, ship it, deal with it later.
But.
AI failure modes are weirder. Bias creeping in through training data. A model that hallucinates contract terms. Decisions nobody can explain after the fact. Even federal agencies are wrestling with this. A recent GAO report on AI acquisitions found that agencies aren’t really collecting lessons learned from past AI projects, which means the same avoidable mistakes keep getting repeated. If that’s happening inside the federal government, with all its compliance scaffolding, imagine how it looks at a 400-person company that hired its first AI lead nine months ago.
Side note, defensive AI use cases tend to mature faster than offensive ones because the risk calculus is clearer. Things like how AI can flag suspicious activity on trading platforms are easier to justify, since you can point at fraud savings on a spreadsheet.
4. Nobody Owns the Adoption Problem
The model ships. Then what?
Often, nothing. No internal champion, no training plan beyond a 30-minute Zoom, and the people who were supposed to use the tool quietly drift back to their old workflow. One user saving time on a task doesn’t, on its own, change a P&L. That requires somebody whose job it is to drag adoption forward, week after week. Most companies don’t staff that role.
It’s possible the next wave works out differently. Then again, people said roughly the same thing about RPA in 2019, and how did that go.
